A Note About Our Physical Therapy Services

Thank you for trusting us with your care. After careful consideration, Primary Care Sports Medicine will end its physical therapy services on December 21, 2026. It has been a privilege to help so many of you get back to doing what you love.

• Starting November 2, 2026, we will no longer schedule new physical therapy evaluations.
• Current physical therapy patients may continue their scheduled visits through December 21, 2026. Please talk with your therapist about your plan of care.
• If you would like a list of physical therapy providers in our area, please call our office and we will be happy to share it with you.
• Your physical therapy records will remain securely on file with us, and you may request a copy at any time.
• Our sports medicine and orthopedic care will continue as usual.
